- How the bodily injury or property damage occurred
- When and where the incident took place
- Names and addresses of any injured parties
- Names and addresses of any witnesses

Providing complete and accurate information helps ensure a timely review and response.
How do I report an incident?
Incident report forms are available through your insurance agent or on this website. Completed forms should be submitted to your agent by email or fax as soon as possible. Your agent will then notify Hospitality Insurance Group.
